Maple Heights lounge receives third coronavirus health order violation citation; Kent bar also cited

CLEVELAND, Ohio – A Maple Heights cocktail lounge has now received its third coronavirus health order violation citation since June after it was visited by the Ohio Investigative Unit and Maple Heights police Saturday.

In addition to the Maple Heights restaurant, a popular Kent bar, 157 Lounge, also received an OIU citation Saturday night.

Vivid Cocktail Lounge, on Broadway Avenue, received citations on June 13 and September 20, according to a news release. The June 13 incident was brought before the Ohio Liquor Control Commission in August, where the owners admitted to the violation regarding the lack of social distancing measures.

The bar received a 15-day suspension of its liquor license, but the owners were also offered the option to pay a $3,000 fine in lieu of the license suspension, information from the Ohio Liquor Control Commission states.

Hawaii rescinds citation given to operator of veterans home

HONOLULU — The Hawaii Department of Health on Friday rescinded an order and citation it issued to the administrator of a veterans home that has experienced a large coronavirus outbreak.


Janice Okubo, state Department of Health spokeswoman, said it rescinded the order and citation to ensure that the documents provided were fully supported in case of appeal.


At least 27 residents have died from a coronavirus outbreak at the Yukio Okutsu State Veterans Home in Hilo. At least 71 residents and 35 employees have contracted the virus from the facility.

Hawaii Department of Health rescinds citation given to operator of Hilo veterans home

The Hawaii Department of Health on Thursday cited the private operator of a state veterans nursing home for deficiencies leading to a COVID-19 outbreak earlier this year in which 27 residents died.

The agency also ordered the administrator of the Yukio Okutsu State Veterans Home in Hilo to implement a list of corrective actions, but then on Friday rescinded both the order and the citation that listed deficiencies in complying with statutory health care regulations mainly related to infection control.

Janice Okubo, DOH spokeswoman, said the rescission was made at the request of the federal Centers for Medicare and Medicaid Services “to ensure that (the documents) are fully supported in the event of an appeal.”

Avalon Health Care, the nursing home’s Utah-based operator, responded to the DOH on Thursday to say it is not admitting fault but that it has taken several immediate actions.
