See Bossier-Caddo restaurants with the most health code violations

The Times obtained food safety inspection information for September 2020 from the Louisiana Department of Health. The Top 7 addresses with the most critical violations are listed, along with an explanation of the finding.

All restaurants in the Top 7 had at least five critical violations.

According to the Louisiana Department of Health and Hospitals, uncorrected critical violations are more likely to contribute to food contamination or illness. Non-critical violations are not directly related to food-borne illness but could become critical if uncorrected.

In total, there were about 775 violations in the region, and approximately 34% of those were critical. About 425 were repeated violations.

More than 345 problems were corrected while the inspector was on site, and many were eliminated by a later inspection.

By parish, violations were:

  • Bossier: 19 addresses inspected — 98 total violations — 35 critical
  • Caddo: 205 addresses inspected — 676 total violations — 225 critical

Most of the critical violations across the region were for:

  • Food-to-food cross-contamination: 69
  • Toxic storage: 50
  • Food contact equipment and utensils cleaned: 22
  • Date marking food: 20

No violations were found at 40 of the 224 addresses inspected. About 100 addresses had no critical violations.

  • Corrected on site: Chemicals were stored with/above food, equipment, utensils, linens, single-service or single-use articles.
  • Flies were present in the establishment.

Sept. 11: No violations found.

Boat Deck 3: 1 violation, 1 critical

Sept. 8:

  • Repeated violation, corrected on site: Drink nozzles were not clean to sight and touch. 

Boat Deck 1: 1 non-critical violation on Sept 8.

Boat Deck 2: No violations on Sept 8.

6. Hy-Lo Mart Deli, 2505 Martin L. King Jr; Drive, Shreveport (11 total violations, 5 critical violations)

Sept. 8:

  • Deli slicer was not clean to sight and touch. 
  • Corrected on site: Chemicals were stored with/above food, equipment, utensils, linens, single-service or single-use articles.

  • Corrected on site: Raw animal food was  not separated from ready-to-eat food, or was placed, stored or displayed above ready-to-eat food. Cooked chicken patties were stored with raw meats.
  • Corrected on site: Raw animal foods with different cooking temperature requirements were not separated to prevent cross contamination during storage, preparation or display.

Sept. 14: No violations found.

5. John’s Seafood Restaurant, 1605 N. Market, Shreveport (11 total violations, 5 critical violations)

Sept. 15:

  • Corrected on site: Chemicals were stored with/above food, equipment, utensils, linens, single-service or single-use articles.
  • Corrected on site: Ready-to-eat, potentially hazardous food prepared on premises and held for more than 24 hours was not date marked. (Rice in cooler._
  • Corrected on site: Raw animal food was not separated from ready-to-eat food, or was placed, stored or displayed above ready-to-eat food in walk-in freezer.
  • Corrected on site: An approved cooling method was not used to meet temperature requirements during cooling.
  • Flies were present in the establishment. 

Sept. 18: No violations found.

4.Griff’s of Shreveport #4 Restaurant, 2920 Old Minden Road, Bossier City (12 total violations, 5 critical violations)

Sept. 16: All corrected on site.

  • Food package was not in good condition, exposing the food to adulteration or potential contamination.  A manager voluntarily discarded flour.
  • Chemicals were stored next to clean utensils. Paint and chemicals were stored above food in the pantry.
  • Using time only as a public health control, food was not marked or otherwise identified with the time within which it would be cooked, served or discarded.
  • Food contact surfaces and utensils are not clean to sight and touch, including inside the ice machine, drink machine nozzles and the can opener.
  • Raw animal food was not separated from ready-to-eat food, or was placed, stored or displayed above ready-to-eat food in the freezer.

Sept. 29:

  • Corrected on site: Raw animal foods with different cooking temperature requirements were not separated to prevent cross contamination during storage, preparation or display.
  • Corrected on site: Raw unprepared vegetables were not separated from ready-to-eat potentially hazardous foods.

Sept. 4: All corrected on site.

Sept. 24:

Big Nate’s BBQ & More: No violations found on Sept. 4.

Blowin Smoke & Kickin Sass: 1 non-critical violations on Sept. 10.

Feeding the Soul Cajun Cuisine: No violations found on Sept. 24.

2. Sam’s Southern Eatery Mobile Unit, 801 W. 70th St., Shreveport (8 total violations, 6 critical)

Sept. 30:

  • Sufficient quantities of potable water were not provided for the needs of the food establishment.
  • Food stored for cold-holding and service was not held at a temperature of 41°F or cooler.
  • A sufficient quantity of hot water was not provided to meet the peak demands in the food establishment.
  • Using time only as a public health control, food was not marked or otherwise identified with the time within which it would be cooked, served or discarded.
  • Employee did not wash hands and exposed portions of arms at appropriate time.
  • Employee was drinking in a food preparation or other area where food equipment, utensils or other items requiring protection were stored.

1. Country Tavern Barbeque Restaurant, 823 Brookhollow Drive, Shreveport (21 total violations, 7 critical violations)

Spt. 25: 16 violations, 7 critical

  • Employee medications stored in the food refrigerator were not stored in a covered, leak proof, labeled container.
  • Ice machine, can opener were not clean to sight and touch.
  • Chemicals were stored with/above food, equipment, utensils, linens, single-service or single-use articles.
  • Repeat: Raw animal food was not separated from ready-to-eat food, or was placed, stored or displayed above ready-to-eat food. Eggs were stored over cheese in walk-in cooler. Bacon was stored with cobbler in walk-in freezer.
  • Employee was drinking in a food preparation or other area where food equipment, utensils or other items requiring protection were stored. Cups without lids were throughout kitchen area.
  • Corrected on site: Food for hot holding and service was held at a temperature of less than 135°F. Cobbler and vegetables were  at 121°F  on the hot line. Time labeling was implemented.
  • Corrected on site: Food stored for cold-holding and service was not held at a temperature of 41°F or cooler. Cheese, coleslaw on the cold line 54°F. Iced , and time labeling was implemented.

Sept. 28: 5 violations, none critical
